Is my privacy protected?
All notes, records, and communication created by Noella Taylor, RSW, concerning your involvement as a client are kept secure and confidential and can only be shared with your written consent. I keep a record of all appointments/visits. There are a few exceptions to confidentiality in which all therapists have a duty to report. In the following rare situations, the law requires that reports be made and/or information released:
- Suspected child abuse or neglect
- Any life threatening danger to a client or to a third party (threat of suicide, threat toward others)
- Reported sexual misconduct by a regulated health care professional
- Court-ordered subpoenas
Noella Taylor, RSW strictly adheres to current legislation governing personal privacy (Personal Health Information Protection Act). All information collected is for the purpose of ensuring accurate contact information, accountability as a service provider, and to ensure that all safety, legal, and ethical standards are met. All clinical documentation and data are stored in a secure location and on a secure server.
Are your services covered by my insurance provider?
The services of Registered Social Workers are covered by most Employee Benefits. Check your benefit package. You will be provided with an insurance-friendly receipt that includes all the information that your insurer requires. This receipt can be submitted to your insurance provider by you for reimbursement. If you do not have insurance coverage, the receipts may be useful for income tax purposes.
What is your cancellation policy?
In order to avoid clients being charged for missed appointments, I require 24 hours notice to cancel or reschedule appointments. I understand that emergencies and extenuating circumstances happen. Please contact me as soon as possible if there is a change needed regarding your appointment.
